In order to apply to enter the competition, this form must be filled in completely and all materials requested must be submitted by August 31st, 2010. There are a limited amount of participant spots in each genre and bands must pre-qualify for competition through submission of this entry form and the media kit items requested. Submission of this form does not guarantee participation in this competition. All artists agree to the Artist Performance Agreement
