Rising Stars Competition
Vendor Form

 

In order to be a vendor at Rising Stars Competition, this form must be filled in completely and all materials requested must be submitted by September 5th, 2011. There are a limited amount of vendor spots and we will attempt to not duplicate types of activities. Submission of this form does not guarantee acceptance. All vendors agree to the Vendor Agreement

Vendors may apply online or print out and mail a copy of the form to the address below

To Apply:

1. Fill out the application on this page or in print. (it will go in the form of an email when you click submit. You must have a default email application in your computer in order for this form to be submitted online!)

2. Mail a copy of the Vendor Agreement along with booth fees to:

This form can be filled out online below, or printed from the link at the bottom of the page and filled out by hand.

By filling out this form, you certify that you are the authorized agent for the organization or company named and that you have the authority to enter into a contract on behalf of the organization.

Please download and print the Vendor Agreement on this page and return it with $40 booth fee to the address on the Vendor Agreement. We will be contacting you with the information for county permit and insurance purposes.

Liability Insurance *
 We have our own Million Dollar Policy 
 We will need to purchase insurance for $22 
The Redwood Empire Fairgrounds requires million dollar liability insurance for every vendor on their property. They must be named in the policy. We will mail separate information with your vendor agreement.
Booth Activity Proposed *
Please describe in detail what activity is proposed for the booth. We will make every attempt to not duplicate activities so it is imperative that we have the details of your activity.
